Patrick Sweet III, M.D.

Patrick Sweet, M.D. is a native of southern California and hails from Ventura County. He is board certified by the American Board of Family Medicine (ABFM), and has had specialized training in dermatologic, endoscopic, gynecologic and musculoskeletal procedures. Dr. Sweet has had additional graduate training in fields complementary to family medicine. Beginning with an internship in Basic Surgery at Naval Medical Center San Diego, he went on to have four years total residency training in Emergency Medicine and Anatomic Pathology prior to completing his residency in Family Medicine. In addition to his patients benefiting from this unique background, Dr. Sweet has a passion for education and has shared his experience with resident physicians as Advanced Procedures Chief Resident at UC San Diego and was awarded the prestigious Society of Teachers of Family Medicine (STFM) Resident Teacher Award. He followed this by being appointed as associate program director for the UC Riverside Family Medicine Residency where he taught and mentored resident physicians and medical students. Most recently he has continued his efforts in San Diego as adjunct faculty at University of San Diego School of Nursing and UC San Diego School of Medicine training nurse practitioner students and resident physicians, respectively. He has written many medical journal articles and book chapters.

Dr. Sweet is a US Navy veteran and served as a medical officer in the active and now reserve components of the US Navy. His military accomplishments include being a plank owner of and deploying with the USS MAKIN ISLAND (LHD-8), team physician to reserve Sea, Air and Land (SEAL) Team SEVENTEEN, Battalion Surgeon to 4th LIGHT ARMORED RECONNAISSANCE BATTALIAN and now as Regiment Surgeon for 14th MARINE REGIMENT. He also holds a volunteer appointment as a medical manager to the Federal Emergency Management Agency (FEMA) and California Task Force Eight (CATF-8) as direct supervisor to Urban Search and Rescue (USAR) medics in the event of a large-scale disaster. For FEMA, he deployed for both the Camp Fire disaster in Paradise, California and Hurricane Harvey in Houston, Texas.

Dr. Luis Bautista, CMO

Dr. Bautista is Board Certified by the American Board of Family Medicine and has been in practice since 2006. In 2001, he graduated t he University of San Diego with a Bachelor of Science in Biology with a minor in Chemistry. Dr. Bautista attended the New York Medical College, graduating in 2005 with a Doctor of Medicine. He completed hisinternship in Family Medicine in 2006 at the Naval Hospital, Camp Pendleton. Dr. Bautista is currently completing his residency in Family Medicine at the University of California San Diego. Dr. Bautista is affiliated with The American Academy of Family Physicians. He has a special interest in sports medicine, dermatology and diabetes. He speaks fluently in English and Spanish.

Dr. Manshi Valluri Mallepogu, MD
Location: Escondido and Santee Family Medicine

Dr. Valluri is Board Certified by the Medical Board of California. She received her Masters of Advanced Studies in Clinical Research from the University of California, San Diego in 2015. She went on to do her Internship at Griffin Hospital, a Yale affiliated Hospital. Dr. Valluri did her Internal Medicine Residency at Mountain View Hospital. She cares about her patients and gets an immense sense of satisfaction when she sees her patients smile.

Dr. Jennifer Kao, MD
Pediatrician
Location: Alpine Family Medicine

Dr. Kao is Board Certified by the American Board of Pediatrics. She graduated from Boston University School of Medicine in 2013, and completed her medical training from Loyola Pediatrics Residency in 2016. Dr. Kao speaks fluently English and Chinese Mandarin. She enjoys cooking, hiking, and traveling in her free time.

Dr. Jacob Singer, MD

Dr. Singer is board certified by the American Board of Family Medicine. He graduated from the Medical College of Georgia at Augusta University in 2010, completed a Transitional Internship at Naval Medical Center Portsmouth in 2011, and completed his Family Medicine residency at Naval Hospital Pensacola in 2015. He joined Mountain Health in January 2019 after serving in the US Navy for 8 years. He enjoys road trips and the outdoors.

Carol Lieber, NP-C
Location: Mountain Health Family Medicine

Carol Lieber is a Board Certified Nurse Practitioner by the State of California Board of Registered Nursing and specializes in Family Medicine. She is also a Board Certified Registered Nurse by the State of Arizona Board Registered Nursing. Carol has been in practice since 1991. In 1991, Carol obtained an Associate Degree from Santa Rose Junior College in Nursing and in 2005 she graduated with honors from the University of Phoenix with a Bachelor of Science in Nursing. She attended the University of Arizona, graduating with honors in 2008 with a Master of Science in Nursing. She holds a National Certification for Family Nurse Practitioners by the American Academy of Nurse Practitioners (AANP). Also, she has a certification in Advances Rural Health. She has worked in a variety of settings ranging from surgical nursing, oncology, home health, to working in a migrant farm worker program. Carol speaks fluently in English and Spanish.

Pam Ferolie, FNP
Location: Alpine Family Medicine

Pam Ferolie is a Board Certified Nurse Practitioner by the State of California Board of Registered Nursing and specializes in Family Medicine as well as a Certified Nurse of the Operating Room. Pam has been in practice since 1980. In 1974, Pam obtained an Associate Degree from San Jacinto College in Nursing and in 1990 she graduated from the University of Redlands with a Bachelor of Science in Business Administration and Management. She attended
the University of San Diego, graduating in 1997 with a Master of Science in Nursing- Family Health Nurse Practitioner. She holds a National Certification for Family Nurse Practitioners by the American Academy of Nurse Practitioners (AANP). Pam is affiliated with the American Association of Operating Room Nurses, American Nurses Association, American College of Nurses Practitioners, California Coalition of Nurse Practitioners, and Sigma Theta Tau, International Nursing Honor Society, Zeta Mu Chapter.


Mikayla Todd, FNP-C
Location: Community Heights

Mikayla Todd is a Board Certified Family Nurse Practitioner with the American Nurses Credentialing Center (ANCC). In 2013, she graduated from George Fox University with a Bachelor of Science in Molecular and Cell Biology. Mikayla attended Azusa Pacific University, graduating in 2016 with a Master of Science in Nursing: Family Nurse Practitioner. Mikayla has a special interest in primary care with a focus in preventative medicine with a holistic approach.

Nicholas Kahl, FNP
Location: Escondido Family Medicine

Nicholas Kahl is a Board Certified Nurse Practitioner by the State of California Board of Registered Nursing and specializes in Family Medicine. Nicholas served in the U.S. Army Medical Corp from 1990-2000 as an Infantry Medic and LVN. He obtained his Bachelors of Science in Nursing from the University of Phoenix and went on to graduate from Ohio State University with a Master’s of Science in Family Nurse Practitioner, graduating with honors from both. Nicholas has been married 18 years to his wonderful wife, Laura, and they have 3 adorable children. He is a HUGE Buckeyes fan, but also enjoys running half-marathons, fishing, wine tasting, and reading classical novels.


Whitney E. Tobin, PA
Location: Alpine Family Medicine

Whitney Tobin is registered Physician Assistant, certified by the National Commission on Certification of Physician Assistants. She obtained her Bachelor of Science in Biology in 2013 and went on to receive her Masters as a Physician Assistant in 2018. Whitney has received the following Certifications: Advanced Cardiac Life Support, Basic Life Support Certified Health Care Provider, and Cardiopulmonary Resuscitation. She has also volunteered as an EMT, at HOPES Free Clinic, and Heartland Hospice. Her Clinical Rotations include; Cardiology, Women’s Health, Emergency Medicine, Internal Medicine, Ears, Nose & Throat, Psychiatry, General Surgery, Pediatrics, and Family Medicine.

Norma Sharpe, PA-C
Location: 25th Street Family Medicine

Norma A. Sharpe is a registered Physician Assistant, licensed by the American Academy of Physician Assistants. PA Sharpe graduated from California State University with her Bachelor’s Degree in Biology in 2001. She is also working towards a Community and Economic Development Certificate. Some of her past activities include the Tulare County League of Mexican-American Woman, Association of Physicians for the Underserved, National Health Service Corps Scholar, Howard Hughes Biomedical Research Program, and was a member of the Health Career Opportunity Program. PA Sharpe’s current and past positions include being a PA for a correctional state facility, worked with at risk youth and adults, and providing services to an underserved population. She is also bilingual, speaking fluent English and Spanish.

Jay Cool, PA-C
Location: Santee Empire Family Medicine

Jay Cool is Board Certified with the National Commission on Certification for Physician Assistants (NCCPA). Jay graduated from Pennsylvania State University Physician Assistant Program in 1979. Served as a corpsman in the USN 1971 – 1975. He has worked in a variety of settings and spent a number of years working for the Department of Veterans Affair.

Joseph Shepherd, PA-C
Location: Mountain Health Family Medicine

Joe Shepherd is a Board Certified Physician Assistant born and raised in East Michigan. He did his training in Grand Rapids, Michigan at Grand Valley State University and graduated in 2017. He is very passionate about health and wellness, loves to surf, snowboard, and play outdoors. He is a current member of California Academy of Physician Assistants and San Diego Society of Physician Assistants. He is also learning to speak Spanish.
