Insurance and Programs

We provide many payment options for your convenience. If you are insured, please bring your insurance cards with you to every appointment. All co-pays are due at the time of service. In the event that you are not able to pay your co-pay at the time of the visit, we will bill you for your co-pay plus an additional $10.00 processing fee.

If you do not have insurance, we can still assist you! Mountain Health can help you apply for public insurances and programs to meet your health care needs. There are many programs which may provide financial assistance, depending on your needs and eligibility. If you do not qualify for one of these programs, you may qualify for a discount under, our Sliding Fee Scale program. If you are interested in applying for these programs, please bring the following information with you to your appointment (all that apply to you):

  • Driver’s license or valid I.D.
  • Proof of residency such as a recent gas/electric bill, telephone bill or mortgage/rent statement with your name and address
  • A recent paycheck stub (a month’s worth)
  • No one who is unable to pay will be denied services
