Welcome to Our Practice!

We are proud to announce that we are currently seeking Patient Centered Medical Home (PCMH) accreditation which is designed to improve delivery methods of patient care. At Mountain Health, we now work in Care Teams consisting of doctors, nurse practitioners, physician assistants and medical assistants in order to give you the highest quality of care. On your first visit, we will encourage you to select a Primary Care Provider (PCP). We will try to schedule you with your PCP whenever possible and if your PCP is not available, we will schedule you with another member of your Care Team. The members of your Care Team share information and have access to your health history in order to better serve you.

Appointments:

Mountain Health is happy to have a centralized call center to handle all patient inquiries. Our qualified call center staff will be able to book and cancel appointments, request contact with your provider on your behalf, and give real-time clinic updates as they happen. Please inform our staff of all health care concerns at the time you make your appointment. Also, let us know if you have been to the hospital or seen another health care provider since your last visit. For all appointments you may call 619-445-6200 and/or any location and our call center staff will be glad to assist you.

What to Bring:

Patients must arrive at the time provided when scheduling their appointment. When arriving for your appointment, please bring the following: Current medication bottles, photo ID, insurance card(s), co-pay and proof of income (if utilizing Sliding-Fee Scale). Acceptable documents for proof of income are Tax Form 1040, or pay stubs (a month’s worth). If you check in late for your scheduled appointment you will be placed on “Walk-In” status.

Walk-in Policy:

Mountain Health strives to meet the needs of all those who enter our facility to the best of our ability. In doing so, we accept walk-ins. The wait time on any given day is directly related to the number of that day’s no-shows or cancellations; however, we will do everything we can to ensure your medical needs are met before you leave our facility. The order in which walk-in are seen is dependent on severity of medical needs.

Same Day Access:

Same day appointments are available which allows access for patients who have an immediate need to be seen on the same day.

Patient Portal:

Mountain Health has built an interactive Patient Portal that will allow patients to access test results, request information directly from their provider, and view upcoming and past appointments. For more information please refer to the patient portal page, from the drop down.

Prescription Refills:

If you need your medication that we prescribed refilled DO NOT CALL THE OFFICE; contact your pharmacy and request the refill. The pharmacy will contact the office for approval if needed. All prescription refills must be called to the pharmacy at least 48-72 hours prior to the actual need of the medication.

After Hours Access Information:

Mountain Health has an on-call provider for patients in need of after-hours access. To reach these services, please call 619-445-6200 and/or any of our offices and follow the prompts. For emergencies, call 911 or go to the nearest emergency room.
