Luis Bautista, MD & interim CMO

Dr. Bautista is Board Certified by the American Board of Family Medicine and has been in practice since 2006. In 2001, he graduated the University of San Diego with a Bachelor of Science in Biology with a minor in Chemistry. Dr. Bautista attended the New York Medical College, graduating in 2005 with a Doctor of Medicine. He completed his internship in Family Medicine in 2006 at the Naval Hospital, Camp Pendleton. Dr. Bautista is currently completing his residency in Family Medicine at the University of California San Diego. Dr. Bautista is affiliated with The American Academy of Family Physicians. He has a special interest in sports medicine, dermatology and diabetes. He speaks fluently in English and Spanish.

Jerry Ayers, MD
Family Medicine and Addiction Medicine
Location: Escondido Family Medicine

Dr. Ayers was Board Certified in Family Medicine by the American Board of Family Medicine in 1987 and re-certified in 1995. He holds an American Board of Addiction Medicine Certification since 2004. Dr. Ayers has been the University of California San Diego with a Bachelor of Science in Biology with a minor in Psychology. Dr. Ayers attended the University of Texas, graduating in 1984 with a Doctor of Medicine. He completed his internship in Family Practice in 1984 at the University of Louisville, Medical School and his residency in Family Medicine at the University of California San Diego, Medical School in 1987. He has a special interest in addiction medicine, family practice, preventative medicine, sports medicine and dermatology.

Dr. Jennifer Kao, MD
Pediatrician
Location: Community Heights

Dr. Kao is Board Certified by the American Board of Pediatrics. She graduated from Boston University School of Medicine in 2013, and completed her medical training from Loyola Pediatrics Residency in 2016. Dr. Kao speaks fluently English and Chinese Mandarin. She enjoys cooking, hiking, and traveling in her free time.

Nurse Practitioners

Carol Lieber, NP-C
Location: Mountain Empire Family Medicine

Carol Lieber is a Board Certified Nurse Practitioner by the State of California Board of Registered Nursing and specializes in Family Medicine. She is also a Board Certified Registered Nurse by the State of Arizona Board Registered Nursing. Carol has been in practice since 1991. In 1991, Carol obtained an Associate Degree from Santa Rose Junior College in Nursing and in 2005 she graduated with honors from the University of Phoenix with a Bachelor of Science in Nursing. She attended the University of Arizona, graduating with honors in 2008 with a Master of Science in Nursing. She holds a National Certification for Family Nurse Practitioners by the American Academy of Nurse Practitioners (AANP). Also, she has a certification in Advances Rural Health. She has worked in a variety of settings ranging from surgical nursing, oncology, home health, to working in a migrant farm worker program. Carol speaks fluently in English and Spanish.

Pam Ferolie, FNP
Location: Alpine Family Medicine

Pam Ferolie is a Board Certified Nurse Practitioner by the State of California Board of Registered Nursing and specializes in Family Medicine as well as a Certified Nurse of the Operating Room. Pam has been in practice since 1980. In 1974, Pam obtained an Associate Degree from San Jacinto College in Nursing and in 1990 she graduated from the University of Redlands with a Bachelor of Science in Business Administration and Management. She attended the University of San Diego, graduating in 1997 with a Master of Science in Nursing- Family Health Nurse Practitioner. She holds a National Certification for Family Nurse Practitioners by the American Academy of Nurse Practitioners (AANP). Pam is affiliated with the American Association of Operating Room Nurses, American Nurses Association, American College of Nurses Practitioners, California Coalition of Nurse Practitioners, and Sigma Theta Tau, International Nursing Honor Society, Zeta Mu Chapter.

Mikayla Todd, FNP-C
Location: Community Heights

Mikayla Todd is a Board Certified Family Nurse Practitioner with the American Nurses Credentialing Center (ANCC). In 2013, she graduated from George Fox University with a Bachelor of Science in Molecular and Cell Biology. Mikayla attended Azusa Pacific University, graduating in 2016 with a Master of Science in Nursing: Family Nurse Practitioner. Mikayla has a special interest in primary care with a focus in preventative medicine with a holistic approach.

Physician Assistants

Shawn Baisley, PA-C
Location: 25th Street Family Medicine

Shawn Baisley is Board Certified with the National Commission on Certification for Physician Assistants (NCCPA). In 2008, he graduated from the University At Buffalo with a Bachelors of Science in Exercise Science. Shawn attended Touro College of Health Sciences, graduating in 2015 with a Master of Physician Assistant Studies. He is affiliated with American Academy of Physician Assistants. Shawn has a special interest in primary care as well as physical & rehabilitative medicine.

Norma Sharpe, PA
Location: Alpine Family Medicine

Norma A. Sharpe is a registered Physician Assistant, licensed by the American Academy of Physician Assistants. PA Sharpe graduated from California State University with her Bachelor’s Degree in Biology in 2001. She is also working towards a Community and Economic Development Certificate. Some of her past activities include the Tulare County League of Mexican-American Woman, Association of Physicians for the Underserved, National Health Service Corps Scholar, Howard Hughes Biomedical Research Program, and was a member of the Health Career Opportunity Program. PA Sharpe’s current and past positions include being a PA for a correctional state facility, worked with at risk youth and adults, and providing services to an underserved population. She is also bilingual, speaking fluent English and Spanish.

Jay Cool, PA
Location: Mountain Empire Family Medicine Jay Cool is Board Certified with the National Commission on Certification for Physician Assistants (NCCPA). Jay graduated from Pennsylvania State University Physician Assistant Program in 1979. Served as a corpsman in the USN 1971 – 1975. He has worked in a variety of settings and spent a number of years working for the Department of Veterans Affair.
